By signing this document, I acknowledge that I have voluntarily chosen to participate in a program of progressive physical exercise at True Performance Fitness. also acknowledge that I have been informed by True Performance Fitness of the need to obtain a physician's examination and approval prior to beginning this exercise program with True Performance Fitness or any type of workout or diet program. In signing this document, I acknowledge being informed by True Performance Fitness of the strenuous nature of the program and the potential for unusual, but possible, physiological results including but not limited to abnormal blood pressure, fainting, heart attack or even death.

• I also understand that I may stop any training session with True Performance Fitness at any time. By signing this document, I assume all risk for my health and well-being and any resultant injury or mishap that may affect my well-being or health in any way and hold harmless of any responsibility, the instructor, facility or persons involved with True Performance Fitness.

Any athlete/member who wishes to participate in this event is able. The Night of the Living Deadlift is an event organized to unite a community for a positive cause. I understand that all profit from this event will be donated under the name of Limitless Physical Therapy, LLC to the Jefferson County Animal Welfare society as well as to deserving graduating senior(s) for education scholarship.

The event will focus around a power lift style deadlift competition in which each athlete will independently choose his/her own weights to enter, under his/her own decision and risk. While the risk of injury that cannot be eliminated, regardless of the care taken to avoid such injuries and the undersigned individual shall assume all risk by participating in the event. There may be food on the premise, and I understand Limitless Physical Therapy, LLC and True Performance Fitness is not liable for any food sensitivities or allergies I have. By signing below, and in consideration of the aforementioned event, I do waive and forever release any and all rights and claims for any damages and liabilities of any kind arising out of my participation in this workshop, against all persons, entities, and agencies involved with performing the event, including but not limited to Limitless Physical Therapy, LLC, True Performance Fitness, and any other related individuals or entities. I understand that this event is not intended to be a sanctioned meet. I also agree to being contacted by Limitless Physical Therapy in the future. We will not share your personal information here onto per Limitless Physical Therapy’s Privacy Policy. I have carefully read this waiver of liability and understand that I am giving up legal rights, including my right to sue. I acknowledge that I am signing this waiver freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the extend allowed by law.
